Enrollment Procedure
To enroll your child at Anna's Bananas Licensed Daycare and Preschool Center, please follow these steps below.
To enroll in Burnsville, please contact the office manager at 952-707-1814.
To enroll in Farmington, please contact the office manager at 651-460-8488.
Registration Fee:
Anna's Bananas has a one-time, non-refundable registration fee of $65.00 per family. This registration fee will need to be paid in the center in the form of cash, money order, Visa, Master Card, Discover, or American Express. Anna's Bananas Daycare and Preschool Center does not accept checks.
Enrollment Forms:
Once your registration fee has been paid, you will receive a link that will lead you to all the enrollment paperwork that is required to be on file at the center and a copy of our parent handbook.
Two forms that are included in your registration packet are required to be filled out by your child's family physician, and a copy of your child's immunization records will need to be submitted to the center prior to first day of care.
